The Area IV Agency on Aging and Community Action Programs, Inc. Care Manager position is responsible for providing ongoing casework for clients receiving services through the Aging and In-Home Services Programs administered by Area IV Agency on Aging and Community Action Programs. The Care Manager is responsible for completing a comprehensive, person-centered assessment to discover strengths, values, and preferences of an individual and identifies all current supports whether formal or informal while incorporating the use of appropriate screening and assessment tools. Assessments will include areas such as housing, income, nutritional needs, informal and formal support systems, health conditions, impairment level, functional needs, mental health status, etc. The Care Manager is also responsible for the development and documentation of a comprehensive, person-centered, and cost-effective care plan with the individual and/or his or her caregiver/informal supports. This process includes identification of desired and available options including informal supports, emergency supports, funding sources, etc.
The position requires An RN with one year’s experience in human services; or a Bachelor’s degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health and Humans Services; or a Bachelor’s degree in any field with a minimum of two years full-time direct service experience with the elderly or disabled (this experience includes assessment, care plan development, and monitoring); or a Master’s degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services may substitute for the required experience.
